Directions: In the table, please indicate the total grant and matching funds PER participant for the Mathematics, Reading and Literacy Supplementary Authorization Incentive Grant Program. Participant may only receive a maximum of $6,000 in support from the grant funds. Also indicate whether the activity will be funded through grant funds and/or local matching funds. All categories of planned matching funds expenditures must also be detailed in the budget narrative. Legislation requires the matching funds to be met by one or both of the following: (1) Cash equal to one third of grant funding received that is to be used in a manner consistent with allowable grant costs; (2) An in kind match of release time or substitute teacher costs for the participating teacher.

Note: Grant funds will be distributed annually in two total payments each fiscal year. The first payment will be 90 percent of the total budget amount, distributed in the spring, and the second payment will be the remaining 10 percent of the budget amount, distributed after the program submits its final data report. An end of year budget form and program report will be collected annually and will be subject to Commission review and approval.
